چکیده

INTRODUCTION The free anterolateral thigh (ALT) skin perforator flap has gained enormous popularity as a free skin perforator flap. Currently being used for various reconstructions primarily in Asia, its routine application for soft-tissue reconstruction of the lower extremity remains unknown in the United States. The purpose of the present study was to review the current trend of flap selection for soft tissue reconstruction of the lower extremity. METHODS We performed a retrospective review of a single-surgeon case log between 2007 and 2011 in a tertiary academic medical center. Inclusion criteria were patients who underwent free tissue transfer for lower extremity reconstruction regardless of sex or age. Exclusion criteria were limited to patients with a body mass index greater than 35 kg/m. A total of 20 patients underwent reconstruction with a free tissue transfer for soft tissue coverage of the lower extremity wound were identified. RESULTS Fourteen free ALT and 6 free muscle flap reconstructions were performed over a 4-year period. Good reconstructive outcomes resulting in reliable soft tissue coverage were achieved in all patients. It was noted that more ALT flaps were used in the last 2 years and less muscle flaps (0/8 vs 6/12, P = 0.042) when compared with the years prior. CONCLUSIONS Although technically more challenging to perform, the free ALT flap has recently been used significantly more for lower extremity reconstruction because of its minimal donor-site morbidity, reliable coverage, and improved contour. This represents a paradigm shift in flap selection that is being highlighted as surgical proficiency in flap dissection technique improves.

Reintroducing the latissimus-rib free flap as a long bone substitute in the reconstruction of lower extremity injuries

Abstract   The leg is a complex district with functions of weightbearing support, stability,   and motility. The management of extensive and complex defects is more challenging   and often results in leg amputation or shortening. Leg amputation is a severe   mutilation that alters the patient’s work and social life by limiting ambulation and   self-sufficiency. During a 3 years period we trea...

Sometimes, because of lack of proper coverage of distal fractured legs, these fractures cause loss of the skin and expose the distal Tibial bones on the medial sides. In such situations, there are two ways for the soft tissue reconstruction: 1. Cross leg flaps and 2. using free flap with microvascular anastomosis. In the present article a case of free Gracilis flap transfer for reconstruction o...

Effectiveness of the Gastrocsoleous Flap for Coverage of Soft Tissue Defects in Leg with Emphasis on the Distal Third

Background: The standard methods for reconstruction of soft tissue defects in the leg include gastrocnemius flap for proximal third defects, soleus flap for middle third and free flap in the distal third. However, there are problems with the use of free flap, like increased operative time, damage of major vessels and the need for experienced microsurgeon. Mathods: This prospective study was und...

Perforator Flaps for Reconstruction of Lower Limb Defects

BACKGROUND Reconstruction of soft tissue defects in the lower third of the leg remains challenging. Anatomical constraints limit the local options available for complex defects especially lower third of leg. Local flaps based on perforator vessels are raising interest in reconstructive surgery of the limbs. We present our experience with perforator flaps for reconstruction of soft tissue def...

Reconstruction of Lateral Mandibular Defects with Soft Tissue Loss: The Role of the Submental Flap

Introduction: Mandibular continuity defects after pathologic resections or traumatic events are difficult cases for reconstruction. Defects involving both hard and soft tissue loss are more challenging, because of problems in soft tissue coverage. The role of the submental flap in this regard is presented.   Materials and Methods: In a retrospective study from the archived files of Ghaem Hospi...
